About Me:

Board-certified in ambulatory care pharmacy, Dr. Chlasta is a clinical pharmacist and Certified Diabetes Care and Education Specialist at Legacy Medical Group - Endocrinology. She earned a Doctor of Pharmacy degree at Belmont University and completed a Postgraduate Year 1 residency at Virginia Mason Medical Center before completing a Postgraduate Year 2 residency in Transitions of Care at Legacy Health. Dr. Chlasta's interests include diabetes management, preventative care, and precepting pharmacy residents and students. Dr. Chlasta is also the director of Legacy Health’s PGY2 Ambulatory Care Pharmacy Residency Program. Outside of work, she enjoys spending time with her family, hiking, and cheering on the Portland Timbers.
