COVID-19 Vaccine

Updated 5/20/21 – By making an appointment today, you can ensure you are fully vaccinated before summer.
It is now fast and easy for people 12 and older, in Oregon and Washington, to schedule appointments directly at Legacy’s mass vaccination clinics. Same-day and next-day appointments are available. Only when more people have been vaccinated can we get back to our normal lives.
Eligibility expands to everyone 12 and older
The FDA has authorized the Pfizer vaccine for kids as young as 12 on May 10th. The company applied to expand Emergency Use Authorization (EUA) for its vaccine for teens and children ages 12 to 15 after a clinical trial showed the vaccine is safe and extremely effective for the age group.
Expanding vaccine eligibility to adolescents 12 to 15 will help us slow the spread of the virus, protect kids and teens in the classroom, at sporting events, during summer activities, and more.

CDC has come out with new masking guidelines
If you are fully vaccinated, you can return to activities that you did prior to the pandemic.
Choosing safer activities.
The CDC still wants masks to be worn on planes, buses, and in hospitals, prisons, and homeless shelters.  Be aware of your own state’s masking guidelines: Oregon and Washington.

All persons 12 and older are eligible for the vaccine. The Pfizer vaccine is the only vaccine that has been approved for ages 12 to 17.


Vaccine sites:

Oregon Convention Center Vaccination Site:

  • Walk-ins welcome
  • 12-14 year-olds must be accompanied by either a parent, guardian or someone designated by the parent.
    • If someone other than a parent or guardian accompanies the 12-14 year old, they will need to provide proof of parental/guardian consent.   
    • Proof of consent is either:
      • A signed consent form
      • A written or typed note that includes the parent/guardians name, relationship to the young adult, their date of birth, a statement saying they consent to young adult being vaccinated and the parent/guardian signature.


Oregon Convention Center will be closing soon. As demand declines and supplies are more widely distributed, some mass vaccine sites will be closing. The Convention Center closure dates are as follows:

  • First doses will continue until May 27
  • Only second doses will be offered in June
  • This mass vaccination site will close after June 19th

The OHA has asked Legacy to participate in medium-sized public vaccination sites to serve specific areas. Those sites have not been determined yet. We will have more specific information to share sometime in mid-June.

There are no changes planned at this time for the mass vaccine sites at Legacy Salmon Creek Medical Center or Legacy Woodburn Health Center. Days and hours might change but both sites will remain open to serve the public and the communities at this time. 

OHSU Drive-through, for those with mobile disabilities:

  • This site is winding down, at the PDX Red Economy Lot.
    • First doses will continue at PDX through June 4.
    • Second doses will be given at PDX through June 25.

Washington Department of Health – vaccine location website:

  • This resource uses locations of pharmacies and health care facilities with vaccine
  • Vaccine availability will vary among locations
  • Each location has its own appointment process and could have its own requirements (such as live in the county where you are making the appointment for the vaccine)


Clark County Fairgrounds vaccination site:

  • Appointments are encouraged online at the Albertsons/Safeway website. Same day appointments are available and walk-ins are welcome.
  • The fairground administers Pfizer vaccine.

Remember to follow safety guidelines

As vaccine doses are distributed across the country in a phased approach, it’s important to continue to follow safety guidelines.

  • Stay home when you are sick
  • Limit group gatherings
  • Distance yourself from others
  • Wash hands frequently
  • Wear your mask
  • Restrict non-essential travel
