Welcome Dr. Warhus as Board Chair

Dr. Warhus’ commitment to provider-led decision-making and value-based care will help guide LHP as the healthcare landscape continues to evolve. When asked what excites him most about this new role, he shared:
"I look forward to gaining deeper insight into the work done by LHP staff, committees, and network clinicians to advance our mission. LHP is well positioned to lead the shift toward value-based care in the Portland metro area."
Dr. Warhus is passionate about LHP’s wildly successful educational initiatives, like the clinical collaboration events on obesity and menopause management, as well as the impact of provider-led quality measures and contract negotiations that minimize the risk of financial harm to clinicians and their practices.
Outside of work, he enjoys road cycling, wine tasting, and spending time with his wife and their two grown children.

Colorectal Cancer Awareness Month: Recent Clinical Collaboration Series event
In honor of Colorectal Cancer Awareness Month, Legacy Health Partners recently hosted an engaging and informative session titled "No Butts About It: Navigating Colorectal Cancer Screening and Prevention" on March 12, 2025.The session featured expert presentations from:
- Benjamin Hornung, MD, Northwest Gastroenterology
- Monina Pascua, MD, PharmD, MSCE, The Oregon Clinic-Gastroenterology
- Alizah Rotramel, MD, FACS, Legacy Medical Group–Colon and Rectal Surgery at Good Samaritan
Attendees gained valuable insights into evidence-based colorectal cancer screening and surveillance strategies, the differences between screening and diagnostic evaluation, and the benefits of colonoscopy versus stool testing. The presenters also shared effective communication strategies to address common patient concerns about colorectal cancer screening.

Attending the live event or viewing the recording will qualify for providers’ learning module credit (be sure to complete the sign-in form at the end) for 2025. Continuing Medical Education (CME) and Maintenance of Certification (MOC) credits can also be claimed until April 11.
Quality Corner: Celebrating Quality Award successes and insights from top practices
Legacy Health Partners (LHP) recently celebrated the achievements of our network's practices through the 2024 LHP Quality Awards. Dr. Albert Chaffin and LHP staff met with clinical teams to learn about the successful implementation of their projects and the key factors that contributed to their success. These award-winning initiatives highlight the importance of collaboration, innovation, and dedication to improving patient care. LHP is proud to share tips, resources, and ideas across our network to foster continuous learning and improvement. By celebrating these successes and learning from each other, we can continue to provide exceptional care for our patients.

Here are some of the valuable learnings from each project:
- Patient outreach for HbA1c: During slower summer months, medical assistants (MAs) at LMG Woodburn Health Center took a proactive role in running A1c clinics for patients with diabetes. Patients were scheduled for nurse visits and staff saw 4-5 patients per day while complex patients went directly to clinical pharmacists. This approach also ensured consistent work for MAs during periods of low physician availability.
- Preventive visits: Oregon Pediatrics’ Wellness Visit Initiative aimed to improve preventive visit rates by utilizing notification tools such as texting, calls, and patient portal messages. The clinic optimized scheduling by holding weekend wellness clinics to accommodate school-aged children and booking wellness checks up to one year in advance to secure spots. Having multilingual office staff also supports diverse patient populations. These efforts increased preventive visits by 11% compared to the previous year, enhancing overall patient care and reducing unplanned visits.
- Patient scheduling: To address the bottleneck in dermatology services, Goodskin Dermatology implemented a self-scheduling system that allowed patients to easily book, reschedule, and cancel appointments. This approach reduced the no-show rate by 12% and achieved a 97% appointment utilization rate, maximizing provider efficiency and patient care.

Announcements, deadlines, and reminders
- In 2025, Legacy Health Partners has twelve contracts managing a shared population of 100,000 lives. All the details about our contracts, including shared savings and care coordination programs can be found on their one-pagers located on the contracting page on the LHP Team Site.
- Thank you to all the adult primary care practices who recently submitted supplemental clinical data from your electronic health record to support 2024 measure results. Our teams are uploading nearly 4,000 additional data points to contribute to measure outcomes and quality scores. Once we have finalized 2024 performance, LHP’s performance dashboards and reporting for 2025 will display in Power BI. We will notify the network soon once the current year’s data is available.
- Have any performance measures to suggest? We want to hear your ideas on performance measures we should consider for the future of LHP. Complete this form to propose a measure. We will accept suggestions throughout the year, but those submitted by the end of April have the best chance of being considered for the 2026 program since we are already planning for next year.
