Legacy pharmacy services provide safe, sustainable, and effective medication management to promote health and healing in the lives of our patients. Legacy pharmacy services strive to deliver an extraordinary patient care experience through exceptional pharmacy services. Our comprehensive services include provision of safe and effective medication therapy for hospitalized inpatients, ambulatory patients in our LMG clinics, apothecaries and Medication Management Clinics. In support of Legacy’s goal to grow into a health management company, Legacy Pharmacy Services strives to meet patient’s medication needs in the appropriate setting.

The past year was unrivaled in its challenges, but Legacy Pharmacy Services came together to rise to those challenges. This commitment was made possible because of the dedication and diligent work of our pharmacists, technicians, support staff, and leadership team.
Pharmacy residency program
Our Legacy Post Graduate Year 1 (PGY1) and Post Graduate Year 2 (PGY2) programs are accredited by the American Society of Health System Pharmacists. These programs provide training to our future workforce and foster an environment of continued learning and growth for our current workforce. We offer six PGY1 positions annually and residents train in five of Legacy medical center sites. We have two specialty residency programs that offer PGY2 training in Ambulatory Care (2 positions) and Infectious Diseases Pharmacy (1 position).
Legacy apothecary locations
Legacy has seven outpatient pharmacies that provide prescriptions, vaccinations, consultations, and over the counter medications for patients at discharge from the hospital (inpatient and emergency room), clinic patients and employees. We also provide access to specialty medications through these apothecary locations. View our various apothecary locations/hours throughout Southwest Washington and Oregon.
Legacy medication management services
With locations at five sites, our clinical pharmacists provide care for patients with complex prescription medications for:
- Anticoagulation (warfarin, enoxaparin and direct oral acting anticoagulants)
- High blood pressure
- High cholesterol
- Hepatitis C
- Patients with 10 or more medications
- Medicare Wellness Visits
A provider referral is required for a patient to be seen in one of our medication management service clinics.
