What is cancer survivorship?
We believe survivorship begins on the day of diagnosis. Cancer affects not only physical but also emotional, economic, social and spiritual issues — it impacts your quality of life. That’s why we offer support and guidance from the beginning of your cancer journey through the end of treatment and into follow-up care. It is all focused on your best health.

What is a survivorship care clinic?
At the survivorship care clinic, our nurse practitioner meets with you to discuss your diagnosis, symptoms, goals and lifestyle and provides recommendations that are backed by research. Your discussion may include:
- Symptom management: Offering ways to ease the symptoms and side effects of treatment, such as sleeping problems, fatigue, anxiety, pain, numbness in your hands or feet, constipation and loose stools.
- Supplement review: Reviewing your over-the-counter supplements and herbal remedies for effectiveness and safety.
- Integrative medicine and complementary care: Referring you to supportive care such as massage or acupuncture. Let your cancer care team know if you are receiving integrative or complementary care.
- Mind/body medicine: Offering techniques and apps that can help you to relax, improve sleep, boost your immune system, and provide many other benefits.
- After treatment and follow-up care: Recommending lifestyle choices to help you stay healthy. Together you develop a plan for life after cancer treatment, and ways to reduce the risk of cancer coming back.
Both hospice and palliative care give support and comfort to maintain your quality of life. However, palliative care is given to patients at any stage of a serious illness or injury. Hospice is for people near the end-of-life who are no longer receiving cure-related treatment. While you might receive palliative care while in hospice, the two are not the same.
What's in a survivorship care plan?
A survivorship care plan is a personalized summary of your treatment and a plan to help you move forward after your cancer treatment. It will help you understand the treatment you received and will include recommendations for the future. It’s a record you share with your doctors.
Your summary will likely include:
- Diagnostic tests and results
- Your cancer type, stage, grade and markers
- Dates, places and types of treatments, and who provided them
- Types of medicine and doses you received
- Side effects you had from your treatment
- Recommendations for future testing and monitoring of your health. This may be a called a surveillance plan.
Your survivorship care plan may be presented to you by your doctor, a nurse practitioner or a nurse navigator. They may also:
- Review ongoing treatment you may need, including medicines
- Provide information about your risk for recurring or second cancer
- Give you a list of symptoms to watch for and notify your provider about
- Offer you resources for emotional support
- Provide recommendations on lifestyle choices you can make to help you stay healthy.
After your treatment is completed, your primary care doctor may take over your care. You may also have follow-up care with your cancer team, so you can find the support you need moving forward.
