Legacy Health’s Diabetes Prevention Program will help you make long-term lifestyle changes to lower your risk of developing diabetes.
Our lifestyle coaches provide a full year of support to help you feel better and keep you motivated. Remember: A family history of diabetes or a prediabetes diagnosis does not guarantee you will develop diabetes. With support from your coach and your group, you can reduce your risk.

Support Healthy Habits
This program promotes modest weight loss through healthy eating, increased physical activity, and stress management, reducing the risk of developing type 2 diabetes.
What is prediabetes?
Prediabetes means your blood sugar level is higher than normal but not high enough to be diagnosed as diabetes. One of three adults in the U.S. has prediabetes and most do not know they have it.
How can prediabetes affect me?
Without making any lifestyle changes, people with prediabetes are more likely to develop type 2 diabetes within five years. This means if you have prediabetes or other risk factors for type 2 diabetes, it’s a great time to start making some changes.
How can I avoid type 2 diabetes?
Making small lifestyle changes–eating healthier, being more physically active, losing weight and reducing stress–will help you avoid type 2 diabetes. Lifestyle changes can cut your risk for diabetes by more than half.
How will this program help me?
You will have the support of a trained lifestyle coach and your peers to make changes and overcome barriers to your success. You’ll discuss topics like shopping and cooking, managing stress, overcoming barriers to lifestyle change, and staying motivated.
How much does this program cost?
Your insurance may fully cover the costs of the program. Still, many insurance companies require a blood test to confirm eligibility. Please contact your insurance company for further information.
When does this program meet?
Groups meet weekly for the first two months, then twice a month until the program’s last two sessions, which are held every three weeks.
