What to do before and after gynecological surgery
In this section we provide general guidelines for preoperative and postoperative care. Please take a few minutes to make yourself familiar with the information and contact your provider’s office with your questions or concerns.Preoperative Preparations for Hospital Surgery: How to Improve the Outcome of Your Surgery
Preoperative preparation is designed to improve the outcome of your surgery, decrease your risk for complications, and make your surgery as safe and effective as possible.In general, no food or drink for at least eight hours before your surgery in the hospital. Most instructions indicate that nothing is to be taken by mouth after midnight, on the night before the procedure. It may seem harsh, not to be able to have a sip of water, but this precaution minimizes the risk for complications such as vomiting during surgery.
If you have diabetes or other pre-existing medical conditions, especially to do with your heart or lungs, let your doctor know and there may be specific instructions on what you should do the day of surgery.
Discontinuing prescription and over-the-counter medications that "thin" the blood, such as aspirin or anti-inflammatories, is necessary prior to surgery. Whether a drug is held or administered is based on the patient's medical condition, the type of drug, and the scheduled surgical procedure. Patients who take prescription medications on a regular basis must discuss this with the surgeon.
If a pain reliever is needed in the week prior to surgery, acetaminophen is recommended over aspirin, ibuprofen, or naproxen, to reduce the risk for heavy bleeding during surgery. If you regularly take any herbal supplement, it may need to be discontinued before surgery.
In many cases, patients are advised to quit smoking 2 to 6 weeks before surgery. Smoking may cause breathing problems during surgery and has been shown to delay healing.
In some cases, a laxative or enema is indicated to empty the bowels before surgery.
If you have any unexpected medical problems before your scheduled surgery, such as a fever, cold, cough or flu please contact the office immediately as your surgery may have to be rescheduled.
General Postoperative Care Instructions:
The goal of postoperative care is to ensure that patients have good outcomes after surgical procedures. A good outcome includes recovery without complications and adequate pain management. Another objective of postoperative care is to assist patients in taking responsibility for regaining optimum health.
After surgery in the hospital
- If you have had general anesthetic, you may have a sore throat for the first 24 hours due to the airway placed in your windpipe. You may also experience dizziness, drowsiness or lightheadedness after anesthesia or sedation.
Incision Care
- If you leave the hospital with bandages over your incision(s) — these bandages can be removed the next day. If you have steristrips (adhesive strips) over your incision(s), they can be removed 7 to 10 days after surgery. If the strips get dog-eared, clip the edges with scissors. Try to keep your incision(s) clean and dry. You may shower 24 hours after surgery. If you get the incision(s) wet, then pat it/them dry. A slight to moderate vaginal flow may be normal for 2-5 days.
- If you have staples in your incision, make an appointment to have them removed within 7 days following your surgery or as directed by your physician. If your stitches are underneath the skin, they will dissolve on their own; you may see a knot at the end and if it is still there at your followup appointment, your doctor will pull it out for you.
Diet
You may return to a normal diet as quickly as you are comfortable doing so. If nauseated, start slowly with clear liquids (jello, tea, broth) and advance gradually to more solid foods.
Activity / Bathing / Driving
- In the first 24 hours after your surgery: do not drive or operate power equipment and do not engage in activities that require coordination for the ability to respond quickly.
- You may walk, ride in a car, climb stairs after your surgery.
- Rest the first day home. You may resume light activity the next day. You may feel weak or tired for a few days after your surgery. Increase your activity as tolerated. Do not do any heavy lifting or vigorous exercise for a week or two if you had a laparoscopy; for 4-12 weeks if you had a hysterectomy or prolapse surgery (as directed by your surgeon). There are no restrictions on activity after a D&C or ablation.
- You may drive when you can control the car fully and are not taking any narcotic pain medication. Please abstain from sexual intercourse until your two followup appointment where further activity instructions will be given. No tub baths, hot tubs or swimming until you see your doctor for your 2-3 week postoperative check. Showers are okay. Activity, such as walking, can help bowel function return to normal and decreases chance of postoperative blood clots.
- Avoid tampon use until you see your doctor for your 2-3 week postoperative visit. No douching.
- You may return to work when you are released to do so by your surgeon.
If you had a hysterectomy: Full recovery can take from 4 to 8 weeks for open abdominal hysterectomy, and from 1 to 2 weeks for vaginal and laparoscopic hysterectomies. During this time, patients should get plenty of rest. Also avoid heavy lifting, tub baths, and intercourse for a full 6 weeks. Depending on the type of procedure used and on the patient's rate of recovery, light chores, some driving, and even returning to work during this period of time may be possible.
Pain Medication
- You will usually be given a prescription for a narcotic pain medication (such as Percocet or Vicodin). You may use this if necessary, however, narcotics are constipating and may make your bowel movements more painful. No driving or drinking alcoholic beverages while taking pain medications.
- Options for non-constipating pain medications include products that include Ibuprofen or Tylenol (acetamininophen).
Preventing and Treating Constipation
- Include fiber in your diet (fresh fruits and vegetables and/or fiber supplements such as Metamucil or Citrucel). Drink at least 6-8 glasses of water each day. Avoid foods that cause you to have gas for upset stomach. Increase activity as tolerated - activity helps bowel function return to normal.
- Take stool softeners as necessary. You may consider using a stool softener (such as Colace) up to 3 times each day if you are taking a narcotic pain medication and/or feel you are constipated. If you have not had a bowel movement within two to three days following your surgery, you may take over-the-counter Milk of Magnesia or Senokot over the counter as directed on the packaging. If you still do not have a bowel movement within 4 days, please call our office.
Preventing and Treating Gas Pains
- Increase activity as tolerated - activity helps bowel function return to normal. Avoid gas-forming foods, such as broccoli, cabbage, beans, salads, sweet potatoes. If gas pain persists, you may take over-the-counter Simethicone (Mylicon, Gas-X, Maalox-Gas, Mylanta-Gas)
Pain Medication
- You will be given a prescription for a narcotic pain medication (usually Percocet or Dilaudid). You may use this if necessary, however, narcotics are constipating and may make your bowel movements more painful. Options for non-constipating pain medications include products that include Ibuprofen or Tylenol.
Follow-Up Appointment
- You should have a follow-up appointment at our office within 2-4 weeks following your surgery. If you don't already have that appointment, please call us at (503) 229-7353 to make it.
Postoperative Care Instructions #1: Colposcopy | LEEP | Cryotherapy
- It is normal to have some mild cramping after your procedure. You may take medications such as ibuprofen (eg Advil — up to 600mg every 6 hours) and/or acetaminophen (eg. Tylenol - take as directed on the bottle) to help reduce the discomfort from cramping.
- You will have some vaginal spotting or bleeding after your procedure. If you had a LEEP you may have a dark coffee grind discharge after that and may pass some tissue in 1-2 weeks. If you had cryotherapy, you may will most likely have a clear vaginal discharge for 4-6 weeks (and may well not have any bleeding or spotting).
- If you had colposcopy and a biopsy, then you are advised to abstain from intercourse, tampon use, baths, and swimming for 1 week. If you had a LEEP or cryotherapy, then you are advised to abstain from intercourse, tampon use, bath, and swimming for 4 weeks. After a LEEP, avoid vigorous physical activity (heavy lifting, etc) for 48 to 72 hours.
- You should have a followup visit in 2 to 4 weeks after your LEEP or cryotherapy (unless otherwise instructed by your doctor). Your followup care after a colposcopy alone will depend on the results from the colposcopy and any biopsies done that day - your physician will call you with the results of biopsies as soon as they are available and review with you whether further action or surveillance is needed (if you have not heard from the office within 2 weeks, please call the office).
Postoperative Care Instructions #2: D&C | Endometrial Ablation | Essure
- You may experience mild to moderate cramping (like menstrual cramps) and pinkish to red vaginal bleeding. This may last approximately 2 to 3 weeks. You may pain medications prescribed by your doctor as needed. In addition you may use ibuprofen (eg. Advil — 600mg up to every 6 hours).
- Use pads for any bleeding. No tampons nor sexual activity for 2 to 3 weeks post procedure.
- Please make sure you continue to use effective birth control.
- Make a followup appointment in the office in 2 to 4 weeks.
Postoperative Care Instructions #3: Hysterectomy
- Full recovery can take from 4 to 8 weeks for open abdominal hysterectomy, and from 1 to 2 weeks for vaginal and laparoscopic hysterectomies. During this time, patients should get plenty of rest.
- Avoid heavy lifting, tub baths, and intercourse for a full 6 weeks.
- Depending on the type of procedure used and on the patient's rate of recovery, light chores, some driving, and even returning to work during this period of time may be possible.
A list of symptoms and complications that can occur after ANY obstetric or gynecologic procedure or surgery
The following symptoms and complications may indicate a medical emergency and should be reported to our office (503) 413-7353 as soon as possible (or proceed to the nearest Emergency Room if necessary):
- Fever of 100.4°F or higher
- Severe abdominal pain not relieved by your pain medication (After laparoscopy, it is not unusual to have shoulder, chest, stomach or lower abdominal pain for 2-5 days. Your pain medication should relieve this.)
- Persistent nausea and/or vomiting and are unable to keep down fluids or food or experience diarrhea for more than 24-48 hours or you do not have a bowel movement within three or four days of your surgery.
- Bright red vaginal bleeding that soaks more than 2 pads in an hour and/or large/painful blood clots
- Increased redness, swelling, or pain around any incision(s)
- Pain with swelling, heat or redness around an IV site
- Pain or tenderness and swelling in the legs, especially back of the calf
- Difficulty urinating or have any signs of a urinary tract infection such as painful or frequent urination, urgency, foul odor or low back pain.
- Difficulty breathing, unusual shortness of breath or chest pain

This office is very well organized, staff knew what to do, medical aide was respectful and knowledgeable about what needed to be done and even offered me use of the restroom. The dr was terrific- she accepted my male friend without question, was thorough and she didn't show any frustration when I didn't remember stuff from 25 years ago. She was gentle and gave me a running commentary on the procedure, not upset when my body reacted and she had to start over-reassured me it was doing its job. Even gave me a phone number for an in-system pcp when I asked. I found all her notes re:my visit on My Health next day - I've never had that before- even noted my friend gave me his hand for comfort. And, I found a complete list of Legacy doctors by clinic! Thanks doctor!

Dr. Hoffmann is, by far, the best Doctor I have ever had. As a medical professional, I would go into OBGYN just to learn and study underneath him. He deserves the highest of recognition.

Dr. Furukawa is professional and very competent as well as forward thinking yet still very personal and easy to work with.
At the start I felt lectured to. She said there was nothing they could do to check for problems with taking the drug Tamoxifen and that I Did Not need to be seen annually. I received the visit summary and then a revised visit summary stating that I did need to be seen on an annual basis. Not sure I'll be back.
Dr. Hoffmann is such an amazing man, he is kind and courteous, he is always considerate and aware of all the health issues going on with me. He is the first OBGYN I have had a decent MD/patient relationship with. He listens to every concern I may have and we talk through it together and make a care plan. This was the best OBGYN experience I have ever had. Dr. Van Houten was clear, kind, put me at ease and did a great job of somehow being thorough and concise at the same time. I appreciated her respect of consent, and her sensitivity. Her demeanor and care was exceptional. I wish I could see her for all my medical needs! The front desk staff and MA Mae were friendly and kind as well.

I've seen Dr. Voge for OBGyn issues for several years now. She's wonderful.
Dr. Voge is one of the best. I look forward to seeing her, which can't be said for many doctors, especially OBGYNs. She's made my life better over the past few years with her care and I feel like I get whole health care even though she's a specialist.
Dr. Emily Edward's was very supportive and helpful when it came to my needs and talking me through it
To me the wait time was long in the exam room but I think it's because I did the e-checkin and arrived early. Appointment length was fine though since Dr. Jensen and I were able to get through all of my questions in 30 min or so.
No one called me about my test results. Experience in person was great, but I received my test results which require follow up and no one has reached out to me. This also happened last year, so your follow up protocol needs a lot of work.
Dr Furukawa is incredibly knowledgeable, skilled, efficient, pragmatic, and kind. I don't know how she does it, but I cannot recommend her highly enough!
Jessica Voge is personable, knowledgeable, and really takes the time to listen and also explain things in a thoughtful way that makes sense.
Overall a good experience. I wish it was easier to schedule an appointment on the providers website. It was not working.

The MA was super nice. She was new, and she was friendly and made me feel welcomed. I hope she stays with the group for a long while. My doctor was amazing, as always. The woman who checked me in was super nice too. There was one woman in check in who I thought was a little rude to someone. The woman came up to be checked in and the receptionist told her, in an annoyed voice, I will be with you when I finish up working on what I am working on. I thought that was an unnecessary way to greet a patient when your job is to actual greet the patient. This was not my experience, however, and everyone was great.
Emily Edwards and her staff are so very nice. It is a pleasure to visit this office.
The doctor and the doctor assistant/nurse were excellent. I felt very confident and comfortable with the information they provided regarding my health concerns. The doctor was attentive to my needs and made sure to give me all the options possible for treatment. She also was very careful when doing her check up on me, which I highly appreciated it. All the staff overall were very kind and the service was very timely.

My partner and I attended our first OB appt w/ Dr Jensen since my usual provider was not available. I was briefly asked about my medical history and then an u/s was performed. Based off my exam, we discovered some questionable concerns. My health history in regards to women's services was not reviewed. I feel this could've helped us understand some of my symptoms if it were reviewed. Dr. Jensen was dismissive of my partner. Asking questions about my partners health history but didn't address him even though he was in the room. Dr. Jensen spent about 15 minutes with us then suggested moving us to another room to do my blood draw. We didn't see Dr. Jensen again after that, just the nurse. My labs were drawn on Wednesday and again on Friday. I was told by Dr. Jensen she would be on call over the weekend and she would contact me with Friday's results. I didn't hear anything. I contacted my usual provider on Monday and received a response same day. I'm sure Dr. Jensen is a good doctor but she is not for me. It almost feels like she wasn't present at my appt. There was no connection, empathy, or compassion in my stressful situation.
Dr. Edwards is a very knowledgable and empathetic provider, and I appreciate how she listens to my concerns, goes over my treatment options, and collaborates with me to create a treatment plan. However, I would appreciate less questions/insinuations regarding my mental health when I'm being seen for unrelated chronic pelvic pain with a history of endometriosis.

The only problem with Legacy is that they over charge for appts. They add extra charges for everything!
She was friendly at first but when I asked questions that disrupted her script she took umbridge and make uncomfortable facial expressions. I then became uncomfortable with her.
Dr. Jensen speaks pretty quickly and seems to think out loud as she considers options. I like to process information before I react, so it made me a bit anxious. In Dr. Jensen's defense, I was anxious about the medical condition for which I was being seen, so I was in a rather sensitive state.
She is too focused on fat shaming me to address my questions about perimenopause. Never acknowledges my healthy habits or years of consistently reducing my weight and BMI despite having an autoimmune disease. Just needs to make sure I know I'm still fat anyway. I've never had a gyn quiz me down about my diet so much. Or my hobbies? Why does a gyn need to know that? Has never acknowledged my perimenopause struggles or tested my hormone levels, but has given me a lot of worthless advice about diet, as if I'm a kindergartener who never heard of a diet or exercise. Does not hear me when I tell her I only eat mostly plant-based whole foods that I cook from scratch and exercise nearly every day. And I'm not sure why I have to go through this every year to get the prescription I need to function. It's very detrimental to my healthy and consistent weight loss journey.
Best office I have been to and Dr.Jensen's holds a great of knowledge and is very professional. Dr.Jensen is respectful and certainly knows what her job well, she great asset to women seeking care.
Apparently there was a problem with the office staff. In particular the receptionists. Two errors were made by them regarding me that sent me t another physician incorrectly and the second required me to make another appointment with this office which should have been taken care of at the visit we are talking about.
It's very difficult to find a great healthcare provider. Dr. Furukawa is excellent.
This doctor was great. My problem is communication with Vancouver Clinic & my ability to contact my primary doctor. It's very problematic: wait times, sending messages , etc.
Dr. Hoffmann is wonderful. His MA is wonderful. They make an unpleasant experience one that doesn't ruin your day. If I have to get a pap I want Dr. Hoffmann to do it.
Dr. Furukawa is an excellent provider. She investigated the imaging results herself and disagreed with radiology's readings, which lead to a new path of healing for me. Her fabulous patient care is comprised of curiosity, creative thinking, intelligence and expertise. I'm grateful for the care she provided for me!
I feel like I left the appointment with more questions than answers. I just was referred to other places for answers because the doctor couldn't answer most involving gender affirming care hysterectomy.

My family has received the best care from Dr Hoffmann, from routine care to a high risk pregnancy I have had wonderful experiences. He has been understanding of all of my needs and concerns and I feel like I was always well informed every step of the way.
I was very impressed with the staff and Dr. Van Houten. I will definitely tell my friends about the excellent experience I had and recommend the clinic.
The receptionist at the front desk that helped get me checked in was incredibly friendly, fast, and helpful. She seemed very good at her job.Dr Edward's is a fantastic provider that seems to care deeply about women's health. She answered my questions thoroughly and advised me on potential next steps with my care.My only negative experience was that I don't think I was given enough time to let the pain meds kick in before getting my iud replaced, or I wasn't given strong enough medication, because it was still incredibly painful.

I love Dr. Voge-she is very caring but also very knowledgeable, and she always thoroughly addresses all my concerns. I'm 58, and she's the best Ob/Gyn I've ever had!

We love Dr. Edwards. My husband and I appreciate the care and attentiveness of her and her staff. We are learning to navigate our first pregnancy and she has been exceptional at explaining what to expect. Her responses are always kind and supportive.
Dr Hoffman and the staff are always friendly and caring and make me feel at ease. Always recommend them to my friends and family who are expecting.
When booking my exam, openings were a bit farther out than I hoped. U did not require the office to relay test results to me because I could access them online. I appreciate the great service and care from Dr. Furukawa.
