This website has information and frequently asked questions (FAQs) for patients who have received a letter regarding a prior Legacy Health sleep study.

The letter notified patients that Legacy found errors in the sleep study interpretation work of a former physician at a Legacy sleep clinic. The physician is no longer employed at Legacy.

I just received a sleep study letter. What should I do?

Please follow the instructions in your letter. If you have questions, contact Legacy sleep study patient services for more information.

Do I need another sleep study?

It depends. If your primary care or sleep provider recommends a new sleep study, then Legacy will cover the cost for one. If Legacy has a primary care provider on file for you, Legacy has notified that provider of this situation.

The first thing you can do is contact Legacy sleep study patient services to discuss your options.

Will I have to pay for another sleep study?

If your primary care or sleep provider recommends a new sleep study, Legacy will provide that sleep study at no cost to you.  

Can my friend or family member call Legacy sleep study patient services on my behalf?

Yes, if you have authorized this person to access your medical information and Legacy has the form on file. Your health information is protected by the Health Insurance Portability and Accountability Act. This is a federal law that safeguards the privacy and security of your health information. You may give someone access to your health information by filling out a Medical Records Authorization Form

What steps has Legacy taken since learning of the issue?

Upon learning about the situation, we launched a comprehensive investigation in accordance with our policies and procedures and the provider was removed from schedules. 

How are you going to prevent this from happening again?

We are always looking for ways to better ensure patient safety. Legacy sleep labs have implemented new software and quality control processes to prevent this situation from happening in the future.  

After this situation, why should I trust you to provide treatment?

Our investigation determined this was an isolated situation involving a single physician who worked as part of other larger teams to provide sleep studies at the Legacy Medical Group–Sleep Medicine at Meridian Park, Good Samaritan and Mount Hood clinics. The provider is no longer employed by Legacy.

Your safety is, and always has been, our top priority. We adhere to robust control practices and take any potential violations of our policies and procedures extremely seriously.

Why wasn’t I notified sooner?

As soon as Legacy became aware of potential errors in sleep study assessments, we began a comprehensive investigation of the situation, and the provider was removed from schedules.

We took steps to carefully identify patients whose sleep study results may have been affected by the work of the former physician and then notified those patients.
