Legacy Member Engagement Program connects new patients with LHP providers

Legacy’s new Member Engagement Program is here! New and unattributed patients in Legacy’s value-based contracts are contacted by our new Member Engagement Program team to help them establish a primary care provider with an LHP practice. The purpose of the program is to create a seamless patient experience for those in value-based contracts, to improve access and to grow covered lives.

Centralized resources, additional one-on-one support and coordination provided by the program benefit both the patient and our practices. The additional engagement with patients also fosters a positive experience and relationship from the very start of their journey with Legacy.

How you can help:

  • Ensure that your Access Report is updated with your Field Operations Advisor monthly. We use this report to understand who has open access for new patients. We also use a randomization tool to ensure that patients are evenly distributed among providers who have access for new patients.
  • Talk with your front desk staff so that they are knowledgeable about the program when the Member Engagement team calls with a patient.
