Antiphospholipid Panel - Antiphospholipid Antibody (AB) - Lupus Inhibitor - Lupus Anticoagulant


PT/PTT, LA Test (Dilute Russells Viper Venom Test or dRVVT)


Relevant patient medical history needs to be submitted with orders for Lupus Anticoagulant testing.  

Electronic orders: Upon ordering, prompts will appear asking about relevant medications such as anticoagulants, antifibrinolytics, recent transfusion history, and history of bleeding or clotting disorders.

NOTE: Anticoagulant therapy, may cause falsely prolonged PT, PTT and LA Test (dRVVT) test results.

PT or APTT mixing studies are automatically reflexed if the PT and/or APTT are unexplainably abnormal (i.e., the patient is not receiving anticoagulant therapy, does not have a vitamin K deficiency, or have a clinical condition that would cause a prolonged clotting time). LA Check (dRVVT) or 1:1 Mix (dRVVT) are reflexed as indicated.  DRVVT denotes Dilute Russells Viper Venom Test.

Other non-clot based tests may be helpful (i.e. Anticardiolipin, Anti-Beta2-Glycoprotein 1) and are included in this panel.

Specimen Collection Requirements


Whole Blood light blue (3.2% Buffered Sodium Citrate)


Refer to Coagulation Specimen Collection Guidelines. Due to short stability, whole blood specimen tubes must reach the testing department within 4 hours, otherwise submit frozen plasma. NOTE: Frost-free refrigerator freezers are not acceptable for frozen sample storage. If proper storage is unavailable at the collection site, freeze/store aliquots on dry ice. Alternatively, send the patient to the nearest Legacy Hospital Outpatient Lab for sample collection/processing.

If submitting whole blood, transport at ambient temperature (15-25°C) in original unopened tubes. If submitting frozen plasma, specimens must remain frozen (-20°C) during transport.

Preferred Volume

Four 2.7 mL whole blood (light blue top) or six 1.0 mL frozen plasma aliquots

Minimum Volume

Three 2.7 mL whole blood (light blue tops) or four 1.0 mL frozen plasma aliquots


Whole Blood: Ambient (15-25°C) 

Plasma: Frozen (-20°C), aliquots must remain frozen during storage and transport

Rejection Criteria

Refrigerated (2-8°C) whole blood sodium citrate tubes or refrigerated (2-8°C) plasma sodium citrate. 

PLASMA: Clotted samples, samples collected in containers other than 3.2% buffered sodium citrate, tubes filled less than 90%, samples from patients with a Hematocrit greater than 55%, hemolyzed samples due to ex-vivo causes (i.e., difficult draw, poor sample draw), whole blood samples that were frozen before processing

PLASMA: Ambient: 4 hours; Frozen: (-20°C) 2 weeks, (-70°C) 6 months
